Also Family Council thanks all those who turned out to the Townhall Presentation on “Optimizing Patient Care at Louis Brier” presented by Leanne Park, Dr. Merrick Tosefsky (Clinical Leads at LBHH), and Dr. Marla Gordon (Medical Director for Long-Term Care & Community Geriatrics Programs at VCH). This important engagement informed us families about the coming improvements to care which are designed to help treat residents here at LBHH where it makes sense to avoid emergency. We hope families review the slide deck to learn what options for treatment will be available in the coming months.

Goals of care conversations which are different from but connected to advance care planning.
Goals of care conversations are discussions LTC Staff have with people living with a life-limiting illness about their goals, values and beliefs so that LTC can align the care provided with what matters most to the person. These conversations support peoples’ understanding of their health now and what it will look like in the future.
Family Councils can best support the conversation by thinking ahead with our loved ones about what matters most. We will touch upon Deciding on Goals of Care and Medical Orders for Scope of Treatment (MOST) Long Term Care.
