Admissions

ADMISSION PROCESS

Choosing a long-term care provider is a difficult decision. Our social worker can help by providing you with more information about long-term care in general and the services we can provide in particular.

Application for all publicly funded complex care beds in Residential Care must be made through the Vancouver Coastal Health Authority.

When you apply for long-term care with the Vancouver Coastal Health Authority, you’ll be asked to choose a preferred facility. We encourage you to find out more about what we have to offer at the Louis Brier Home & Hospital, and to call us for a tour, if you so desire, to see in firsthand everything we have to offer. That way, you can determine if Louis Brier is your preferred residence.

We’re the only long-term care residence in Vancouver providing traditional Jewish celebrations and cultural activities, kosher food and professional care rooted in the values of Judaism.

Levels Of Care Available At Louis Brier

Louis Brier provides 24-hour nursing and personal care to all residents; the amount and type of daily care provided to residents differs based on the level of care needed.

Intermediate Care
Provides a private room and three kosher meals per day plus snacks. 24-hour nursing coverage is provided as well as medication management and assistance with some personal care. Residents in Intermediate Care are still mobile and able to get to activities and the dining room for meals.

Extended Care
Provides a higher level of care with more nursing staff to assist residents. In addition to the support provided in Intermediate Care, residents in Extended Care also get help with all personal care, mobility and nutrition. Extended care has both private and semi-private rooms.

Special Care Unit
Our small 17 bed Special Care Unit is for those residents with advanced dementias who need a smaller, more intimate environment that provides safety and security for those who are no longer able to navigate a complex environment.

How To Apply For Residence

Application for all publicly funded complex care beds in Residential Care must be made through the Vancouver Coastal Health Authority. Louis Brier Home & Hospital neither creates nor maintains a waitlist.

If you’re interested in applying for residency at Louis Brier, you must first call Central Intake of Vancouver Coastal Health at 604.263.7377.

You’ll be assigned a Long Term Care Case Manager from your local Community Health Centre (also known as Public Health Unit), who will assess your care needs.

Admission From Hospital

The process for admission from hospital may be similar to that for individuals living in the community. When hospital staff deems the patient unable to safely care for him/herself post discharge and for the long term, the Transition Services Team (TST) will assess the patient as described above and waitlist him or her for the first available bed in the community.

Please note that Louis Brier is unable to offer admission to anyone at any time without permission from Priority Access.

Please contact our Social Worker, Limor Shamy at 604-267-4744  lshamy@louisbrier.com to learn more about the admission process or to schedule a tour.
