FALL PREVENTION PROGRAM
Our Falls Prevention Program has been created based on Best Practice guidelines. Upon admission, every resident receives a full assessment of fall risk, considering many factors including, but not limited to:
- Current mobility level
- History of falls
- Use of assistive devices (walker, canes, eyeglasses, hearing aids)
- Medication used to prevent fractures and bone loss
- Cognitive function
The Falls Data collection ensures that resident safety is promoted throughout the organization and all members of the interdisciplinary team are held accountable. Appropriate intervention strategies are reviewed and discussed amongst the interdisciplinary team on which approach is best to manage falls for a specific resident.
